OUR SERVICE
Community outreach is a service that cares for individuals with autism spectrum conditions, learning disabilities, behaviours that may challenge and emotional needs within their own homes and in specialist services in the Burton area. Individuals are supported through a person-centred planning approach which is tailored towards individual needs and focused on outcomes. Each person is encouraged to take part in meaningful activities in order to enrich their lives and increase their independence. Additionally, people are supported to identify and maintain any education placements, volunteering and/or work experience. Staff work with individuals, health care professionals and family members in order to provide the best care.

We understand that there are times when we all need a little extra support; whether it’s help around the house, personal care, medication or mobility support, your carer is committed to providing support that brings you peace of mind.
Home and family care from our community outreach support workers offers that extra support, reassurance, and companionship that may be needed for you to continue living independently at home.
But home care is not only confined to the home. Your support worker will support you to get out and about to enjoy your hobbies and interests as you always have. They can even accompany you on holiday or visiting your beloved family and friends, so there really is no compromise to your social activities and lifestyle
Each individual is encouraged to lead an active life and take part in a variety of activities both within the service and within the community. These include activities such as trips to the cinema, gardening, swimming, bowling, day centre activities, day trips to various surrounding area and meals out to name a few. Staff are happy to accommodate any activities that the individual requests to do, also supporting families at home, social support and domestic assistance, home sits.
